What's the Difference?
These three types of care are often confused, and it's understandable why. They overlap in some ways but serve very different purposes. This guide will help you understand each one clearly so you can make the most informed decisions for your loved one.

​Hospice Care
Hospice care is a specialized form of care for individuals who have a life-limiting illness and whose physician has determined that curative treatment is no longer the focus. It is not about giving up — it is about shifting the goal from treating a disease to honoring a life.

Hospice care focuses on:

Managing pain and physical symptoms so the resident is as comfortable as possible

Providing emotional and spiritual support for both the resident and their family

Coordinating all aspects of end-of-life care through an interdisciplinary team

When should hospice be considered?

Hospice may be appropriate when a resident is living with a life-limiting illness such as advanced dementia, cancer, cardiac disease, lung disease, or kidney disease — and when the focus of care has shifted to quality of life and comfort. Specific indicators may include:

Uncontrolled or increasing pain

Significant decline in functional ability or appetite

Increased shortness of breath or difficulty swallowing

Repeated hospitalizations without sustained improvement

Oxygen dependence or progressive weakness
Medicare, Medi-Cal, and most private insurance plans cover hospice care. Our team can help your family understand your coverage and navigate the process.


UNDERSTANDING YOUR CARE OPTIONS
Palliative Care
Palliative care is often misunderstood as being the same as hospice. It is not. Palliative care is specialized medical care focused on providing relief from pain, symptoms, and the stress of a serious illness — and it can be provided at any stage of illness, alongside curative treatment.
Think of palliative care as an added layer of support that works in partnership with whatever treatment your loved one is already receiving. A resident can receive palliative care while also receiving treatment for cancer, heart disease, dementia, COPD, or any other serious condition.

Palliative care is led by a physician who specializes in this field and delivered by an interdisciplinary team that may include doctors, nurses, social workers, counselors, and chaplains.
Coverage: Medicare, Medi-Cal, and many insurance plans cover palliative care services. Veterans may also be eligible through the Department of Veterans Affairs. Our team can assist your family in confirming coverage.​

Home Health Care
Home health care brings skilled medical and therapeutic services directly to your loved one — whether that means within our Glen Park community or in a private home setting. It bridges the gap between hospital or clinic-based care and daily life, providing professional healthcare support in the environment where a person feels most comfortable.
Home health care is appropriate when a resident needs:

Skilled nursing care following a hospitalization, surgery, or change in health status

Physical, occupational, or speech therapy

Ongoing wound care or medication management

Chronic disease management for conditions such as diabetes, heart disease, or COPD

Assistance with daily activities provided by a trained home health aide
Medicare and most insurance plans cover medically necessary home health services when ordered by a physician. Our team will help coordinate referrals and communicate directly with your loved one's care providers.

Steward Hospice Care brings a holistic approach to end-of-life support, attending not only to physical comfort but to the emotional, spiritual, and practical needs of residents and their families. Home Health Care Partners

Steward Home Health Agency provides flexible, responsive home health services with a focus on whole-person care. Their multidisciplinary team works closely with physicians, specialists, and our Glen Park care staff to ensure continuity and quality across every aspect of a resident's health plan. Their inclusion of palliative and hospice support alongside skilled nursing and therapy services makes them a versatile and valued partner.
Services include: Skilled nursing care, physical, occupational, and speech therapy, home health aide support, chronic disease management, medication management, wound care, telehealth services, medical social services, palliative and hospice care, and patient and family education.

Allegiance Pharmacy

Glen Park Senior Living partners with Allegiance Pharmacy, a long-term care pharmacy that understands the unique demands of assisted living and memory care environments. Allegiance was selected not only for their clinical expertise and operational reliability, but because their values — transparency, resident safety, and a commitment to continuous improvement — mirror our own.
They are fully licensed, maintain compliance with all applicable state and federal pharmacy regulations, and work in close coordination with our care teams across all seven Glen Park communities.

Single Bubble Pack Medication System — Each resident’s medications are pre-packaged in individual, clearly labeled bubble packs organized by resident, date, and time of administration. This system virtually eliminates the risk of dispensing errors, ensures consistency in medication delivery, and makes the verification process fast, clear, and auditable. Each pack arrives ready for administration — no sorting, no guesswork, no room for error.

Electronic Medication Administration Record (eMAR) — Glen Park utilizes an electronic medication administration record system that provides real-time documentation of every medication pass. Our medication technicians record each administration digitally at the point of care, creating an accurate, time-stamped record that is immediately available to our licensed nursing staff, community management, and — through Allegiance Pharmacy’s platform — to the pharmacy team. This level of transparency and accountability means that any missed dose, change in prescription, or clinical concern can be identified and addressed immediately.

Seamless Physician & Family Coordination — Allegiance Pharmacy works directly with each resident’s physician to process new orders, refills, and medication changes efficiently. Families who prefer to use an outside pharmacy may do so; our team will coordinate the transition into our medication administration system. We are committed to making this process smooth for everyone involved.

Certified Medication Technicians — All medication administration at Glen Park is carried out by state-certified Medication Technicians trained in the safe and accurate dispensing of both prescription and over-the-counter medications. A licensed nurse is on site daily across our communities for additional oversight and clinical support.
