Assisted Living

Assisted living is for older adults who value their independence, but need some assistance with daily activities such as bathing, dressing, transportation and medication reminders. Seniors may live in their own suites or with a companion and are relieved of household chores such as laundry, cooking, and cleaning.

In addition, the Glen Park team offers an Individualized Service Plan (ISP) to personalize each resident’s assisted living care needs.

Memory Care

Glen Park’s memory care services are focused firmly on the individual. Dementia slowly interferes with one’s mental ability to manage everyday life.

Our memory care services create a safe and stimulating environment for those with dementia–including its most common form, Alzheimer’s.

Hospice Care

Glen Park partners with Five Star Home Hospice, Inc. and Steward Hospice Care, Inc. to help those in our care and their families to live as fully as possible when facing the end of life. We believe regardless of age, race, handicap, sex, sexual orientation or national origin, that everyone has the right to control how they choose to live at the end of life. After all, this is a time in life when Moments Matter Most!
