2022 Rates
*Basic Services - 3 meals daily; 3 snacks, daily housekeeping, laundry, daily activity program, field trips, all utilities except private or cell phone, escorting to meals and activities, furnished unit, schedule doctor appointments, wellness checks, safety checks and medication management.
Level of Care of Services: A personalized assessment will be completed by our professional staff to determine the appropriate Care Level for all prospective residents. Memory Care assessments consider the behaviors and base line for redirection by staff to ensure safety and quality care. The assessment will be reviewed and discussed with the resident/responsible party.
Note: Facility phones are available for residents to receive and/or make calls for free of charge. Facility provides transportation to and from medical appointments and to pick residents up from the day program if needed during normal business hours.
Nursing Services: Includes checking vital signs, blood sugar testing, blood pressure checks, insulin administration, wound care for stage 1 and 2 prescribed by residents’ Primary Physician or Specialists that are not covered by Residents’ insurances.
Behavioral Services: Incudes ongoing Behavioral Assessment and Intervention Plan which will be developed by our House Board Certified Behavior Analyst. Intervention Plans will only include positive and proactive techniques and strategies that will increase the exhibition of desirable, acceptable, and functional behaviors while reducing the barriers attributed to the exhibition of previously learned behavior. This will increase the likelihood that the client will be able to establish and maintain positive and rewarding social relations and experiences. Our Behaviorist will determine the needs and frequency of visit for each resident that requires the service.
Transportation Services: Transportation for our Activity related programs is free. Otherwise, we charge $100 per visit for any medical and/or dental appointments within 7-mile radius. We will assist residents with arranging transportation and companionship for free. We do not transport beyond 7-mile radius.
